dc.description.abstractLa autonomía del paciente es un principio central en la atención médica, ya que le permite participar activamente en decisiones sobre su salud, favoreciendo la satisfacción, la adherencia a los tratamientos y una mejor relación médico-paciente. Respetarla implica reconocer la dignidad y los derechos humanos, garantizando que cada persona pueda decidir sobre su propio cuerpo y bienestar. Esto fortalece la confianza, la comunicación y la alianzaterapéutica, promoviendo un rol más activo del paciente en su cuidado. Sin embargo, la autonomía no siemprese ejerce de manera plena: puede verse limitada cuando el paciente no comprende realmente la información, toma decisiones precipitadas o se encuentra bajo estados de dolor, pánico o fatiga que distorsionan su juicio. Surge así un dilema ético cuando un paciente capaz rechaza un tratamiento vital, lo que enfrenta la obligación médica de preservar la vida con el respeto a la autodeterminación y al consentimiento informado. Patient autonomy is a central principle in contemporary medical practice, enabling individuals to actively participate in decisions regarding their health, thereby promoting satisfaction, treatment adherence, and a stronger physicianpatient relationship. Respecting autonomy acknowledges human dignity and fundamental rights, ensuring that each person can make decisions about their own body and well-being. However, autonomy is not always fully exercised; it can be limited when the patient does not fully understand the information, makes rash decisions, or is under states of pain, panic, or fatigue that distort their judgment. This gives rise to an ethical dilemma when a capable patient refuses a vital treatment, pitting the medical duty to preserve life against respect for selfdetermination and informed consent. Through a reflective analysis, this article explores the tensions between patient autonomy and professional responsibility, emphasizing the importance of dialogue, clear communication, and an ethical approach to navigating these complex clinical situations.
