    Lactancia materna, desarrollo motor y obesidad, ¿Existe asociación causal?

    Childhood obesity is the main nutritional and public health problem in Chile, being the principal causes, the increase in energy dense foods and the decline of physical activity. Interventions to prevent obesity at infancy are focused mainly in improving quality and quantity of dietary intake, without taking into account physical activity, which is expressed under two years of age, mainly by motor development. Some studies have proven that motor development at early age, may influence the ability to perform physical activity. Thus, infants scoring a lower motor development may have a greater risk of becoming obese. It isn’t know if childhood obesity causes lower motor development (given that children may have greater difficulty to move), or on the contrary, it is the lower ability to move, which increases the obesity risk. The objective of this manuscriptis analize the evidence regards the relation between breastfeeding, motor development and obesity in the childhood.To be able to understand this asocation and casual mecanism, it is important to develop stategys focused in early infancy to promote breastfeeding, healthy eating and early stimulation, starting in pediatric office.

    Neck Circumference Cut-Off Points for Identifying Adiposity: Association with Chronic Metabolic Diseases in Older People

    Background: The leading cause of death in older people is cardiovascular diseases. Several studies have found that neck circumference (NC) is a simple anthropometric marker associated with adiposity. The aim of this study is to estimate and validate NC cut-off points as adiposity markers and analyze their association with cardiovascular and chronicmetabolicdiseasesinolderpeople. Methods: Across-sectional study in 358 non-disabled, community-dwelling older people (71.7 ± 3.9 years) living in Santiago de Chile and participating in the HTSMayor study was conducted. Measurements of body composition and cardiovascular risks were evaluated. Receiver operating characteristic (ROC) curves and multiple logistic regression models were used to evaluate the association of NC with cardiovascular and chronic metabolic diseases. NC cut-off points were obtained to predict obesity, abdominal obesity, and adiposity. Results: The best performance values of neck circumference relative to obesity and adiposity were obtained with respect to abdominal obesity (40.6 cm in men and 34.2 cm in women). Higher NCvalues wereassociated with a higher area under the curve (AUC) for men and women(men: AUC=0.84; women: AUC=0.86). NCwassignificantly associated with a higher risk for diabetes mellitus (OR = 1.95), hypertension (OR = 2.42), acute myocardial infarction (OR = 4.36), and comorbidities (OR = 2.01), and a lower risk for sarcopenia (OR = 0.35). Conclusions: This study shows that NC is a useful tool for detecting abdominal obesity, obesity, and adiposity in older people and that a higher NC increases the risk of chronic diseases.

    Trayectoria del desarrollo psicomotor según estado nutricional en niños alimentados con lactancia materna

    Objetivo: Describir trayectorias del desarrollo psicomotor (DSM) y nutricionales en niños alimentados con lactancia materna (LM) y evaluar asociaciones. Sujetos y Método: Cohorte prospectiva de lactantes nacidos a término, sin patologías conocidas, alimentados con LM predominante o exclusiva. La ingesta de LM se evaluó con técnica isotópica a los 3 meses. En 6 edades sucesivas se evaluó estado nutricional según índice de masa corporal (IMC) y DSM mediante Ages and Stages Questionnaire (ASQ-3). Se incluyeron niños con al menos 2 evaluaciones de seguimiento. Se realizó análisis multivariado de las trayectorias por dominio o área del DSM según IMC y LM, controlando por variables bio-socio-demográficas. Resultados: Ingresaron 53 niños (60% hombres), 76% eutróficos, 62% alimentados con LM exclusiva. Las trayectorias nutricionales y DSM fueron estables y en rango de normalidad, no hubo lactantes en rango de obesidad. En el análisis multivariado, en el dominio Comunicación, los niños con una trayectoria IMC normal con valores más altos y con LM exclusiva presentaron desarrollo superior (p = 0,049 y p = 0,032). En el área Resolución de Problemas, tener trayectorias de IMC normal con valores más altos se asoció a mejor trayectoria del desarrollo (p = 0,040). No se encontraron asociaciones significativas en las trayectorias de dominios motrices y nutricionales. Conclusión: Las trayectorias nutricionales y del DSM fueron estables y en rangos de normalidad. La LM exclusiva en comparación con la predominante se asoció con mejor trayectoria en Comunicación, mientras que tener valores altos de IMC dentro de la normalidad se asoció con mejor trayectoria en Comunicación y Resolución de Problemas.
